 note = {鹿児島県は28の有人離島が存在し，その中には医師が常駐していない離島も多数ある。我々は鹿児島県の準無医地区離島である三島村と協同で高齢者介護予防事業に取り組む機会を得た。ここでは，準無医地区離島の高齢者に対する介護予防システム構築までの経緯と介護予防事業の効果について報告することを目的とした。介護予防システム構築には三島村民生課と協同で介護予防事業計画を策定し，看護師，作業療法士（OT），理学療法士（PT）が主体となって在宅での運動指導を実施した。運動指導における高齢者の反応は良好で４年間における離脱率は４%であった。さらに高齢者57名を対象に３カ月間の運動介入の効果を検討した。各対象者に対する身体機能評価から在宅で簡便に実行可能な運動プログラムを立案・実施した結果，日常生活活動（ADL）や生活の質(QOL) に有意な改善が認められた。今回，三島村と協同で高齢者介護予防システムを構築した。また，医療資源の不足する離島において看護師と定期的なOT，PTによる運動指導や生活指導により介護予防が実行可能であり，ADLやQOL改善効果が期待できることが示唆された。, In the Kagoshima prefecture, there are 28 people dwelling isolated islands, however, the physician is not stationed in the most isolated island. We obtained an opportunity to take part in the preventative care project for the elderly in cooperation with Mishima-mura, which is an isolated, doctor-less island in Kagoshima prefecture. The aims of this report were to describe the process of developing a community based elderly preventative care system and to investigate the effect of this preventative care service. Firstly, we devised a preventative care project in cooperation with the Mishima-mura public civil affairs division. In this preventative care project, the home-based exercise programs was performed to elderly by a nurses, occupational therapists (OT), and physical therapists (PT). The withdrawal rate of this preventive care service during 4 years was approximately 4%, suggesting that the participants were almost satisfactory. Secondly, we investigated the effects of the 3 months exercise intervention in 57 elderly people dwelling isolated islands. As the results, the activity of daily living and quality of life were significantly improved by the exercise intervention. In conclusion, we developed an elderly preventive care system in isolated, doctor-less island. In addition, it provides evidence on feasibility and efficacy of this preventive care service by nurse, OT and PT for improving the physical and mental health.},
