The patient was in his 60s and had undergone renal transplant, but did not adhere to his medication regimen. The following were extracted from electronic medical records: (1) The patient’s progress with treatment, descriptions of symptoms, and the patient’s comments during hospitalization, (2) Nursing care and support, (3) Contents of care conferences, and (4) Pharmacists’ records of interventions. Effective educational interventions were discussed by the nurses according to Prochaska’s “transtheoretical model of health behavior change”.\nResults: Precontemplation: The patient requested that his wife assist with medication management; however, nurses explained the necessity of self-medication to the patient. Contemplation: Although he accepted the nurses’ explanation, he made many mistakes. Nurses repeatedly taught him how to take his medicine each day. The patient’s situation was shared with other nurses, doctors, and pharmacists at a conference. Preparation: He continued to make many mistakes with medicine names and doses. However, the patient became determined and proposed a better way to prepare his medicines. A nurse tested this method with his wife. Action: He was pleased with his life without dialysis.
