We performed pancreaticoduodenectomy with D3 lymph node dissection. Metastatic lymph node tissue was submitted to chemosensitivity test. Based on the chemosensitivity test, FOLFOX (Leucovorin + 5-FU + Oxaliplatin) was administered intravenously. The patient received 3 cycles of FOLFOX chemotherapy until appearance of grade 4 leucopenia and grade 3 vomiting. After the FOLFOX, she received 12 cycles of TS-1 + CDDP chemotherapy without serious complications.  She remained well without any symptoms and pursued normal activity for 27 months.
