The SP-LI was significantly higher in the hernia group (5.49±3.01 pg/ml) than in the control group (2.05±0.52 pg/ml) (p<0.01). In the hernia group,the SP-LI was significantly higher in patients with severe pain in the lower extremities than in those with only mild pain. As the SP-LI was found to be correlated with the severity of pain,it was considered to be au seful index of pain. As for the correlation of SP-LI with the hernia type,the SP-LI wass ignificantly higher in patients with transligamentous extrusion type hernia than in those with protrusion type hernia. This result suggested that the release of substance P was increased with marked compression on the dorsal root., 腰椎椎間板ヘルニア患者40例と腰痛や下肢症状を有さない対照者10例について,脳脊髄液のsubstance P様免疫活性(SP-LI)をradioimmunoassayにより測定した. 腰椎椎間板ヘルニア患者では脳脊髄液中のSP-LI濃度は5.49±3.01pg/mlと対照者の2.05±0.52pg/mlに比べ,有意に高かった(p<0.01). また腰椎椎間板ヘルニア患者において,下肢痛が重篤なほど, SP-LI濃度は有意に高かった. これより脳脊髄液中のSP-LI濃度は疹痛の重症度と相関をもち,疼痛の客観的評価の指標になり得ると考えられた. ヘルニアのタイプとSP-LI濃度との関係は,trans-ligamentous extrusion typeのヘルニアを有するもののSP-LI濃度がprotrusion typeに比べて有意に高かった. これよりヘルニアによる後根の強い圧迫でsubstance Pの放出が増加し,下肢痛が増悪していると考えられた. 医歯学総合研究科博士論文(医学) ; 学位取得日: 平成8年3月15日}, title = {腰椎椎間板ヘルニアにおける脳脊髄液中のSubstance P}, year = {1996}, yomi = {サメシマ, コウジ} }
