| 内容記述 | 目的：腎移植患者の，内服管理の行動変容につながった看護師の関わりを明らかにする。 方法：対象は生体腎臓移植を受けた60代男性。電子カルテから，（1）治療経過と患者の発言，（2）看護ケア，（3）医療者間カンファレンスの内容，（4）薬剤師の指導記録を抽出した。患者の言動と看護師の介入をProchaskaの行動変容ステージモデルに従って検討した。 結果：無関心期：患者は妻に内服管理を依頼したため，看護師は自己管理の重要性を説明した。関心期：薬剤名前や作用，量の間違いが多かった。看護師は，薬剤師と薬名などを説明した。医療者間カンファレンスで情報共有し，指導法を再検討した。準備期：薬名や量の間違いは多かったが，自ら方法を提案してきたため妻と共に試みた。実行期：間違いは減少し，看護師は妻と患者の努力を褒めた。 結論：腎移植患者の内服自己管理の介入は，患者の意欲を確認しつつ，家族や医療スタッフと協力しつつ行うことが欠かせない。 |

| 内容記述 | Purpose: The purpose of this study was to analyze the aspects of educational interventions by nurses that produced behavioral changes related to self-medication management in a patient who underwent renal transplant. Methods: Case report. The patient was in his 60s and had undergone renal transplant, but did not adhere to his medication regimen. The following were extracted from electronic medical records: (1) The patient’s progress with treatment, descriptions of symptoms, and the patient’s comments during hospitalization, (2) Nursing care and support, (3) Contents of care conferences, and (4) Pharmacists’ records of interventions. Effective educational interventions were discussed by the nurses according to Prochaska’s “transtheoretical model of health behavior change”. Results: Precontemplation: The patient requested that his wife assist with medication management; however, nurses explained the necessity of self-medication to the patient. Contemplation: Although he accepted the nurses’ explanation, he made many mistakes. Nurses repeatedly taught him how to take his medicine each day. The patient’s situation was shared with other nurses, doctors, and pharmacists at a conference. Preparation: He continued to make many mistakes with medicine names and doses. However, the patient became determined and proposed a better way to prepare his medicines. A nurse tested this method with his wife. Action: He was pleased with his life without dialysis. Adherence to his medication regimen was established and the nurses acknowledged his effort. Conclusion: Educational intervention related to self-medication management for a renal transplant patient requires cooperation with families and medical staff while confirming the patient’s motivation and intention. |
