| 内容記述 | 本研究は，中小規模病院の医療従事者側と患者側双方の医療に対する姿勢について調査し，医療従事者・患者の安全文化の医療安全への影響について病院機能に注目して検討を行うことを目的とした。中小規模病院3施設（一般病院A，Aと同法人のリハビリテーション病院a，異なる法人の一般病院B）の病棟に勤務する看護師133人を対象に，“Hospital Survey on Patient Safety Culture”の12因子と，看護師から見た患者の積極性についての自記式質問紙調査を実施し，病院ごとの特徴についてはANOVAを，またイベント報告数への影響は重回帰分析を用いて検討した。その結果，病棟レベルの安全文化に対して法人全体の組織的な取り組みの影響が大きいことが示された一方で，病院レベルの安全文化は，病院の機能により決定される部分が多いことが示された。また医療安全や安全文化が良好な病院では上司の態度が医療安全の向上に影響を促すことが示唆された。 | |||||||||||||||||||||||||

| 内容記述 | PURPOSE: The purpose of this study was to examine safety culture and patient activation, i.e., the level of patients’ activeness in their own medical care, and the effects of these determinants on patient safety in medium-sized hospitals. METHODS: The Hospital Survey on Patient Safety Culture (12 factors), which was administered to nurses, and the level of patient activation as rated by the nurses were measured, and ANOVA and multiple regression analysis were performed to examine the aspects of these scales and the effects on the number of adverse events reported. RESULTS: The unit level safety culture was affected by the culture of the medical corporation and the hospital level safety culture was affected by the role of the hospital (general hospital or rehabilitation hospital). “Frequency of Event Reporting” and the level of patient activation were positively correlated with the number of adverse events reported by general hospital nurses. “Supervisor/Manager Expectations & Actions Promoting Safety” was positively correlated and the level of patient activation was negatively correlated with the number of adverse events reported by rehabilitation hospital nurses. CONCLUSION: In a medium-sized general hospital that is developing patient safety, a good attitude by the nursing staff and a high level of activeness of patients in their own medical care may improve patient safety. In a medium-sized rehabilitation hospital with developed patient safety, a good attitude by the nursing manager may improve patient safety. |
