内容記述 Introduction: Mercury (Hg) exposure during pregnancy is an important issue due to its ability to pass the placental blood barrier leading to adverse health effects on the fetus. Fish consumption is considered to be one of the major sources of exposure to Hg in the general population. The association between low level of Hg exposure in pregnant women with birth outcome and child growth development is controversial.
Objective: In this study, we evaluated prenatal Hg exposures in pregnant women and compared the differences in Hg concentrations between pregnant women living in highland and lowland areas in Semarang, Central Java Indonesia. Furthermore, we examine the relationships between the mother’s hair Hg concentration with birth outcomes and postnatal growth and development of children at 6 months of age.
Method: In 2018, 118 pregnant women were recruited from six different community health centers located in the northern coastal area in Semarang, Central Java Indonesia. Their children were followed up until 6 months of age. Data on the characteristics of the mothers during the pregnancy, birth outcomes, and child growth and developmental status were collected. Information on fish consumption was collected using a questionnaire. Total Hg concentrations were determined from hair samples. Data were analyzed using Mann-Whitney test, Kruskal-Wallis test, and regression analysis.
Results: The median (range) of the hair Hg level in the study subjects was 0.434 (0.146 – 8.105) ppm, and three subjects had more than 2.8 ppm (above the upper limit reported by the Ministry of Health, Labor and Welfare, Japan). Pregnant women who lived in the lowland areas, near the sea, showed a higher hair Hg concentration than those living in highland area [median (range) : 0.465 (0.146 – 8.105) vs 0.385 (0.150 – 1.956) ppm; p = 0.053]. Fish consumption in lowland areas was also higher compared with that in highland areas [(85.71 (0 – 500.0) vs49.76 (0.0 – 428.57) g/day; p < 0.01]. The multivariable regression analyses showed a positive association between maternal hair Hg and birth weight (regression coefficient = 154.6; standard error = 115.0; p = 0.182), and no effect on birth length (regression coefficient = -0.089; standard error = 0.721; p = 0.902), after adjusting the effects of potential confounding factors. Regarding the growth of children at 6 months of age, the multivariable regression analyses revealed a positive trend between maternal hair Hg and children's weight at 6 months (regression coefficient = 97.51; standard error = 270.8; p = 0.719), and a similar effect on children's height (regression coefficient = 0.067; standard error = 0.757; p = 0.929). There was no association between maternal hair Hg and developmental score of children at 6 months old.
Conclusion: The hair Hg concentration of pregnant women in Semarang, at low range, was not associated with the birth outcomes and growth-developmental status of children at 6 months of age.
