| 内容記述 | 目的：無医島における糖尿病看護の実態を調査し、現状を明らかにすることである。 方法：鹿児島県の無医島3島のへき地診療所に勤務する全看護師7名および役場保健師1名にインタビューガイドを用いて半構造化面接を行い、質的・記述的に分析した。 結果：無医島に居住する糖尿病患者に対して島で実践している看護として、【多職種連携】【患者・家族への直接的な支援】【システムに関すること】【島のコミュニティ】の4つのカテゴリと20のサブカテゴリが抽出された。 考察：無医島では巡回診療や島民の自宅訪問に加えて、インフォーマルな場における看護活動が実践されており、へき地診療所看護師の特徴的な看護活動であると考えられる。また、他職種の不在により多職種連携が困難であり、糖尿病患者に対する継続支援が十分ではないと考えられる。 結論：物的・人的資源の不足する無医島において、糖尿病患者を継続的に支援するためのシステム整備が必要であり、ICT を活用した遠隔看護や多職種連携の充実が望まれる。 |

| 内容記述 | Objective: To investigate the current state of diabetes care nursing on remote islands without doctors to clarify the existing situation. Methods: Semistructured interviews were conducted with all seven nurses and one municipal public health nurse working at remote clinics across three islands without doctors in Kagoshima Prefecture, Japan. Using an interview guide, the data collected were analyzed qualitatively and descriptively. Results: Four main categories (interprofessional collaborative practice [ICP], direct support for patients and families, system-related matters, island communities) and twenty subcategories were identified as nursing care provided to patients with diabetes living on the islands without doctors. Discussion: On remote islands without doctors, nursing activities include not only mobile healthcare services and home visiting, care but also informal settings that are characteristic of remote clinic nurses. Due to the lack of other healthcare professionals, ICP is challenging, and ongoing support for diabetic patients is perceived as insufficient. Conclusion: On doctorless islands with limited material and human resources, there is a need to develop a system that provides continuous support for diabetic patients. Enhancing telenursing and ICP through information and communication technology is essential. |
