| 内容記述 | 我々は2015年から「地域での暮らしを最期まで支える人材養成―離島・へき地をフィールドとした教育プログラム」を行っている。その中で，訪問看護師へのシャドーイングにより在宅看護過程を履修生に追体験させる実習をしている。本研究の目的は，今後の在宅看護を担う人材育成に資するために，臨地施設に看護職として勤務している履修生28名の本実習終了後のレポートを質的帰納的に分析し，対人的援助関係の過程を基盤とした思考過程である看護過程展開上の特徴を明らかにすることである。 その結果，1. 自己の価値観で対象を理解している，2. 対象の言動の本当の意味を理解できていない，3. 医学的思考に偏った看護を展開している，4. 対象の尊厳を尊重しない看護を展開している，5. 対人的援助関係がうまく築けない，という履修生の特徴が明らかになった。 これらは，在宅看護を担う人材育成の際に，留意すべき課題であり，再教育の必要性を示唆している。 その際，プロセスレコードを用いて自己の在宅看護過程を再構成させ丁寧に振り返らせる（リフレクション）という方法は非常に有効である。 |

| 内容記述 | We have been providing intervention for remote islands and areas based on an education program for human resource development to support all residents’ community lives since 2015. During training for visiting nurses as part of this program, participants reflect on their home nursing process using the shadowing technique. With the aim of nurturing professionals for future home nursing, we qualitatively and inductively analyzed reports, submitted by 28 nurses with clinical experience after such training, and examined the characteristics of their nursing process. Five characteristics were identified through analysis: 1) recognition of patients based on each nurse’ s own values; 2) insufficient understanding of patients’ true intentions through their statements/behaviors; 3) development of nursing driven by medical thoughts; 4) development of nursing without sufficient respect for patients’ dignity; and 5) difficulty in establishing favorable patient-nurse relationships. These points should be noted when developing human resources for home nursing. They also suggest the necessity of re-education for these nurses. At that time, the method of reflecting using the process record is very useful. |
