目的：日本の健康増進に関する歴史的変遷を可視化し、課題を明らかにすることを目的とする。方法：D財団の健康増進に関する公募研究論文に基づいて、国民健康づくり運動を基準に４区分して計量テキスト分析した。結果：第１期は、健康づくりの推進や基盤整備・普及を重視する２次予防中心の対策だったので、疾患の解明に重点を置いた調査が中心であった。第２期は、運動習慣の普及等の１次予防へと政策の焦点が移されたことにより、支援教育と行動変容を主とした研究がみられた。第３期は、生活習慣病克服のための数値目標が設定され、成人病の予防や治療に関する内容が多くなり、マクロ的・ミクロ的な基礎研究へと向かっていく時代に入った。第４期は、新たな健康課題や社会背景等を踏まえながら政策が推進されており、健康増進の個人の責務に言及していた。結論：健康に関する研究テーマの変遷は、健康増進の施策の変化を反映していた。また、健康問題は個人の努力で解決するには難しいものもあり、社会レベルでの取り組みが必要である。 |

PURPOSE: In this study, we sought to visualize the historical changes in health promotion policies in Japan and to identify issues surrounding them. METHOD: We performed a search of open-access research articles on health promotion published by Foundation D, organized the articles into categories based on the four historical phases of Japan's national health promotion movement, and performed quantitative textual analysis on the content of each article. RESULT: The first phase of Japan's national health promotion movement consisted of policies aimed mainly at secondary prevention of diseases, with an emphasis on promoting health and establishing and expanding the necessary infrastructure. The articles in this category therefore tended to focus on research to clarify disease mechanisms. The second phase of the national health promotion movement saw a policy shift toward primary prevention of disease by the encouragement of regular exercise, so the articles in this category mainly described support for education and behavior modification. The third phase of the movement established numerical targets for eradicating lifestyle diseases; accordingly, many articles in this category addressed the prevention and treatment of adult-onset diseases, signaling a shift in basic research from a macro perspective to a micro perspective. The fourth phase promoted policies incorporating new health issues and social contexts, so the articles in this category noted the obligations of individuals to improve their own health. CONCLUSION: The shift in health research themes over the years has reflected changes in government policies for health promotion. Moreover, our findings highlight the difficulty of addressing health issues solely through individual efforts, and suggest the need for initiatives at a societal level. |
